We are deeply grateful for the trust you have placed in us to nurture your children's faith and fellowship through our church events and travel opportunities. Your support means the world to us and to the young hearts in our care. Thank you from the bottom of our hearts for allowing your students to join us in these meaningful experiences.

To ensure everything runs smoothly and safely, we've prepared two simple consent forms for your convenience:

• The top form serves as an annual permission for our regular day-to-day youth activities, covering ongoing participation with ease.

• The bottom form is tailored for our exciting day trips and overnight adventures, providing the necessary approvals for those special outings.

If you find it easier, feel free to complete this online form, save a copy to your device, print it out, and simply have your student hand it to a youth leader or email it directly to us at First Baptist. We're here to make this as straightforward as possible for you!
